      Effective immediately, the new simplified Term and Universal Life Application packet is available for print or download from www.myprotective.com. The packet only includes basic forms, state required forms and a few optional forms for single sided print only.
      Please note: this simplified application packet does not include some of the optional forms that may be required depending on your submission. Other forms must be printed or downloaded separately when necessary. (The full application packet – including supplemental forms – two sided print – remains available for print or download.) In addition, the new simplified Application for Conversion or Exchange is also now available on the My Protective website. Login and select “Doing Business” and click on “Forms and Applications”. (The old application form will continue to be accepted until everyone’s supply is exhausted.) Enhancements to the Policy Conversion and Replacement Information sections of the Application for Conversion or Exchange eliminate the need to submit a full application with evidence of insurability questions that do not apply.
