    The Coverage Continuation Rider will no longer be available for new sales of ANICO Executive UL SI. Important deadlines are:

    • February 28, 2017 is the last date an application can be written for an Executive UL SI policy with the Coverage Continuation Rider.
    • March 10, 2017 is the last date an application can be received in the home office in order to retain the Executive UL SI with the Coverage Continuation Rider.

The Rhode Island Secondary Addressee Notice (ORD112674 RI) edition 2017 is required to be provided to the applicant at point of sale.
  • Revised HIV Consent Form for Iowaeffective February 27th
    Iowa recently amended its Administrative Code to include updated CDC national health information hotline contact number and website address. The contact information is used to provide further information on HIV testing and AIDS. On February 27, 2017, the revised Iowa Notice and Consent Form for AIDS Virus (HIV) Testing form (ORD88624 Iowa) edition 2/2017 will be available. Prior versions of the form will not be accepted after that date.
